Sargent Art Group – Membership Application Form
There are two versions of the Membership Application Form. The information asked for is identical for both versions. Choose one. First I made a fillable-savable PDF, which is great. However, instructions are a little complicated for people who are not computer proficient. If you are not computer proficient, scroll down to Version 2 and fill in the blanks on this page.

Instructions for filling out Version 1 of the Membership Form: (I am using Windows on a PC. If you are using a Mac or IPad, I can't be of any help to you.) Click on "Download File". If you are using Chrome or Edge, the form should open and you can type in your responses.
If using Firefox, the form opens and you don't see a blue color above the blanks, you need to click on the download icon (which looks like a down arrow on a file folder) located just above the white form on the dark background on the upper right. Click on that and choose to open in Adobe Acrobat Reader.
Type your info into the blue spaces, save the form to your computer. Click here on [email protected] which should open your email program. Type "SAG Member Form" in subject line. Attach your filled form. (Click on icon that looks like a paper clip, navigate to the file on your computer which is the form you filled out. Click on the file name. Click on Open.) Once your filled form is attached, click Send (or you could write me a note telling me how easy or difficult it was for you).
